We Collect The Following Information :
 

1. Name of client

2. Address of client

3. Date of birth
    

4. Medical, medication & allergy disclosure.

This is recorded as a hard copy consultation document which is stored safely in a locked cabinet for 7 years. After this time the documents will be destroyed.
No electronic copies are made so there is no online threat to your details. You can request what information we hold about you if you have been a client. This information will be sent to you as soon as possible and no later than 1 month after initial request.
If you choose to cancel the treatment contracts with Caroline Day, We still have to hold on to the client record cards for 7 years. No Data will be shared with 3rd parties.
